About the Role

As a Care Home Care Coordinator, you will play a critical role in ensuring that each resident in the care home receives outstanding, person-centered care. The position is designed for individuals who are passionate about the well-being of elderly residents and want to contribute positively to their daily lives. You will manage the care planning and delivery processes, ensuring that the residents’ individual needs are met with empathy, professionalism, and dedication.

You will work closely with our experienced care team to coordinate and manage care delivery. This role is essential in maintaining the smooth operation of our care services, from planning and organizing shifts to administering medication and updating care plans. The ideal candidate will have a passion for elderly care, coupled with excellent leadership and organizational skills.

Key Responsibilities

As a Care Home Care Coordinator, your responsibilities will be broad and varied, but will primarily focus on ensuring that every resident receives the high-quality care they deserve. Here’s a closer look at what this role entails:

  1. Coordinate and Manage Resident Care:
The primary responsibility of the Care Coordinator is to manage and oversee the care services provided to our residents. You will work closely with the care team to assess the needs of each resident and ensure that the care they receive is tailored to their individual preferences, goals, and medical requirements. This includes organizing regular meetings with care staff, doctors, and other healthcare professionals to review each resident’s progress and adjust their care plan as necessary.

  2. Shift Management:
A critical part of the role is to ensure that shifts are well-organized and that the care team is fully staffed at all times. You will be responsible for overseeing shift schedules, ensuring that there are no gaps in coverage and that each shift has the appropriate number of staff to provide the best care. This may involve adjusting rosters, managing absences, and ensuring that staff are well-supported and prepared to perform their duties effectively.

  3. Medication Management and Record Keeping:
Administering medication is a key part of the care provided to residents. As the Care Coordinator, you will ensure that all medication is administered correctly and that accurate records are kept. This involves maintaining digital care plans for each resident, ensuring that all information is up to date and that any changes in medication or health status are promptly recorded. The role requires a high level of attention to detail and a strong understanding of the medications being administered, as well as the ability to identify any potential issues.

  4. Communication with Healthcare Professionals:
Effective communication is essential to ensure the delivery of comprehensive care. You will liaise with external healthcare professionals, such as doctors, nurses, and specialists, to provide coordinated care for residents. This will involve attending meetings, preparing reports on residents’ health and well-being, and ensuring that all parties involved in the residents' care are kept informed of any changes.

  5. Staff Training and Supervision:
Another significant aspect of this role is to assist in the induction, training, and supervision of new and existing care staff. You will be responsible for ensuring that all staff members are well-trained and up-to-date with the latest practices in care provision. This will involve providing hands-on training, overseeing staff performance, and offering support and guidance as needed. Strong leadership skills and the ability to mentor and motivate your team will be essential in ensuring that residents receive the best possible care.

About You

This role is suited for a compassionate and organized individual who is experienced in supervising care teams and is passionate about providing person-centered care to elderly residents. The ideal candidate will have excellent communication and leadership skills, a deep understanding of residents’ needs, and the ability to manage the complexities of care coordination in a fast-paced environment.

Specifically, we are looking for someone who:

  • Experience in Supervising Care Teams:
You should have prior experience in supervising or managing a care team, ideally within a care home setting. Experience in care coordination, shift management, and overseeing medication administration is highly desirable.

  • Empathy and Understanding of Residents' Needs:
A successful candidate will be someone who has a deep sense of empathy for elderly residents and can work collaboratively to meet their individual needs and preferences. You must be able to build strong relationships with both residents and their families, providing reassurance and comfort during difficult times.

  • Strong Leadership Skills:
The ability to lead a team effectively is crucial. You must be able to work well as part of a team but also take charge when necessary, ensuring that all staff members are performing their duties to a high standard.

  • Organizational Skills:
This role requires an organized and methodical approach, particularly in managing shifts, care plans, and medication. Attention to detail is essential, as is the ability to manage multiple tasks simultaneously.

  • Flexibility:
You will need to be flexible in your approach to work, including being open to working different shifts, taking on additional responsibilities as needed, and adapting to changes in residents’ care needs.

Qualifications

To be considered for this role, you must hold, or be working towards, an NVQ Level 3 in Health & Social Care. This qualification is essential for providing high-quality care and ensuring that all care practices adhere to industry standards. If you are currently working towards this qualification, we will support your continued professional development and encourage you to achieve your certification.
